Reshapes the lower eyelids by removing or repositioning excess fat and tightening loose skin to smooth under-eye bags and achieve a more youthful look.

Introduction

Introduction

The lower eyelids, together with the cheeks, play a crucial role in framing the eyes and are often where age-related changes such as bags, grooves, and shadowing first become noticeable. Dr. Kim’s Lifting Lower Blepharoplasty secures sagging under-eye fat to the periosteum, filling in deep hollows to achieve a seamless and soft under-eye line. When paired with our anterior cheek and aegyo-sal (pretarsal roll) lifting, the result is a naturally bright and revitalized appearance.

Procedure Overview

Procedure Overview

The procedure involves an incision of the attenuated orbital septum with minimal skin excision to maintain structural integrity. Orbital fat is either excised or repositioned to flatten deep tear troughs, followed by fixation to the infraorbital rim and lateral canthal periosteum for sustained durability. Simultaneously, elevating the sagging mid-cheek and precisely excising excess skin allows this comprehensive approach to shorten the lower eyelid and restore youthful midface volume, creating a naturally refreshed, radiant look.

The incision for lower blepharoplasty is strategically placed to ensure optimal access while keeping scars discreet and virtually invisible. In most cases, the incision is made just below the lower lash line (subciliary approach), following the natural contour of the eyelid crease.

Benefits

Benefits

Lower blepharoplasty delivers transformative rejuvenation by addressing under-eye bags and puffiness, instantly creating a more rested and alert appearance. Through meticulous removal of excess skin and strategic mid-face elevation, the procedure shortens the vertical height of the lower eyelid while restoring youthful cheek volume, achieving a seamless, harmonious transition from eyelid to mid-face.

This refinement smooths the tear trough, diminishes deep shadows and “tired” lines, and produces a firmer, more even under-eye surface, making makeup application effortless. Beyond the visible aesthetic enhancement, patients benefit from improved facial symmetry and enduring confidence, enjoying a naturally refreshed, balanced look with minimal, discreet scarring.

Ideal Candidates

Ideal Candidates

This refined procedure is designed for the individual who wishes to erase the appearance of persistent fatigue by addressing prominent under-eye bags and deep shadows. Rather than a simple correction, this advanced approach targets under-eye laxity, volume loss, and the hollowing between the lower eyelid and cheek to restore a smoother, more youthful mid-face contour. By improving skin firmness and refining the transition from the lower eyelid to the cheek, the procedure enhances facial balance while creating a more even, resilient surface that feels comfortable in everyday movement and supports effortless makeup application. Ultimately, this treatment is tailored for those who demand invisible precision, providing a naturally radiant and harmoniously balanced look with scars that remain discreetly hidden. Dr. Kim will perform a comprehensive evaluation of the eyelid anatomy, skin quality, and overall facial balance, and will discuss personalized goals and expected outcomes to ensure you can make a fully informed decision before proceeding.

Recovery

Recovery

A few hours after your procedure, you will be discharged from the clinic and can return to your accommodation accompanied by a friend or family member. As Dr. Kim performs the surgery under sedation rather than general anesthesia, most patients feel comfortable afterward. You will be prescribed appropriate pain relief, NSAIDs (Nonsteroidal Anti-Inflammatory Drugs), antibiotics, and anti-nausea medication. We recommend keeping your head elevated for the first 48 hours of rest, even while sleeping, to promote optimal healing.

For exercise and sports, Dr. Kim advises avoiding strenuous activity for the first two weeks, with gym workouts and sports postponed for an additional two weeks, as excessive movement may interfere with healing and increase the risk of bleeding. Also, our clinic also offers one of the best adjuvant therapies including healing lasers, LDM, IV therapy, and HBOT for optimal results and recovery. Post-op skincare regimens are essential to maintain outcomes, and Dr. Kim provides patient-specific advice as standard, included in the procedure price.

Expected Timeline Days 1-2: Rest with head elevated at about 15 degrees, cold compresses for swelling control. Mild discomfort managed with medication. Days 3-7: Swelling peaks then subsides. Light activities (walking, desk work) resume. Stitches removed on day 7. Weeks 2: Most swelling gone. Return to daily/social activities, light exercise allowed. After 6 weeks: Full recovery. Final results visible, optimal outcomes sustained. Cold compresses, head elevation, avoiding heavy exertion and smoking are essential.
